Fill the Biodata Form: Visit the link provided and fill out the biodata form with your accurate details. Note that the matriculation number should be typed without the inclusion of slashes (‘/’) or hyphens (‘-‘). For example, a correct matriculation number format would be 991992A006.
Upload Your Picture: Graduating students are expected to upload a recent passport photograph with a white background. The photograph should be without eyeglasses or caps.
Save the Document with Your Matriculation Number: After filling out the biodata form and uploading the picture, students are expected to save the document with their matriculation number. For instance, a correct saving format would be 039B02AV009.
Print out the Document: Once the document has been saved correctly, students are then expected to print it out.
